Why is Worldwide cover more expensive than Europe?
Leaving aside other factors such as age, pre-existing medical conditions and trip length, the difference in price mostly comes down to the cost of medical treatment in different countries.
Take a week's course of antibiotics for an ear infection. In Greece it might cost around €6, while in the USA you could pay about $50. When treatment costs several times more, claims are higher, so cover for those destinations costs more.
The same applies within Europe. Treatment in Switzerland usually costs more than in France, for example, and not every country in Europe accepts the GHIC.
